Liability Disclaimer

In consideration of being permitted to participate in roller derby as part of Volcanic City Rollers (VCR), I agree that:

• I am physically and mentally fit (or have discussed my status with the League Coaches, as per page 1) and am able to participate in the sport of roller derby.

• I am advised and acknowledge that participation in the sport of roller derby involves inherent risks, dangers and hazards that can result in injury to myself, serious or otherwise. I acknowledge there is a possibility that I may be injured during the course of practice and / or games.

• I am aware and acknowledge that participation in the sport of roller derby (whether in practice and / or games) may result in damage to personal property (any property owned by me, in my possession or under my control).

• I will indemnify Volcanic City Rollers against all claims, damages or losses (including costs) I may incur as the direct or indirect result of any injury / damage to myself or my property.

• Neither I, nor anyone associated or connected with me, will make any claims against Volcanic City Rollers in respect of any injury suffered by me; or any damage to my property regardless of how the injury or damage occurs.

I consent to the details contained in this form to be held by Volcanic City Rollers. I acknowledge my right to access and correct this information. This consent is given in accordance with the Privacy Act 2020.
